Why aren't the Booleans from my model showing up like the example below

I think you have put the DCA write on the wrong line item. I don't think you need to have DCA set for Submit Purchase Order Request. It should be on the next line item so that once you submit your request, then your Suggested Order Amount for the Month shows up.
