I am facing error in location code and my demand values are invalid
Plz tell me if there is error in dem04 module or not

Compare the time you see in your data (Jan 20) vs what you are mapping it against (2019-01). You need to go to time tab in your mappings and update it (to something like MMM YY). This would solve problem 1.
Second is in your mappings, map column headers to line items and then go to line item tab and make sure Demand is mapped to Volume.
Should work then. Happy to get on a call if that is quicker.
