Converting Data within module

I have data in below format.

I am expecting to create a new module with below dimension and structure. Could you please suggest an efficient way for the same?

    Create an Employee List and an Options List

    Create a module dimensioned with these 2 lists with a single line item for Values. Then import the data from your file into this module. Map the employees and options as columns and use column header mapping for line items. Then map the Value column to the Value line item
