I don't see the modules "COLA 01 COLA Rates" and "COLA02 COLA by Employee" existed in the example 1 module given. Are these modules downloadable somewhere?

Hello, Can you share a snippet of the list of modules? Additionally you can right click on any on the percentage & select "drill down" and it'll show you the source module. On top left there'll be a drop down to open the source module.
I believe that has to be one of example module name for understanding the LOOKUP concept
