Real to Fake Timescale

I am trying to map the Anaplan monthly timescale to a fake timescale that I have created. 

 

I have a location that has a date MM/DD/YY in which I am wanting to map that to my Month list that consists of Month 1 through Month 60. I am trying to do a formula that will map that date as the Month 1 list item and then each month after that number it Month 2 - Month 60. I have gotten it to map the Month 1 to the actual month, but I am stuck trying to get the months after that to map. Could someone help me?

Do see if the following works:

 

1. Create Fake Mth as shown below. I only created 24 members but you can expand to 60 members.

2. Cost Center Date Module is created to capture Date of 1st month.

3. Another module, i.e. 'Date to Fake Mth' is created. Results are shown in 'Fake Month (List)'.

4. Blueprint of 'Date to Fake Mth' module.
