Catering for Jagged Hierarchies

Just ramping up on lists and from my previous experience many clients have jagged hierarchies (i.e. there is not a consistent level of parent-child relationships).

Just looking at composite lists these require the hierarchy to have a consistent level. How would jagged hierarchies be catered for?
