Help with Formula

Hi,

Need help creating a Formula that will bring in a Date from our time setting based on a Boolean LI for that specific Date.

Below is a screen shot to better understand the question:

When the Boolean is checked we need to be able to look up the date associated with it.

We then want to be able to pull in that associated date into this LI that is located in a different Module.
