I have mapped Display Name as well, and this is happening in one list only while else other list working fine.

What is the format of the Display Name? In this instance, it should be Text.

And do you have line items referencing the Display Name? If so, remove those formulas just to get the data in.
No Display name is not referred to anywhere.
Remove the Display Name from the definition. And, based off the name, the data is coming from a csv?

I feel it happens sometimes, and gives this error, and if you change the item uniquely identified to Name and then change it back to code, surprisingly, it will create the action.
