@Ankur.Sonthalia can you please check for data consistency, as somewhere it is in format DD-MMM-YYYY and somewhere DD-MM-YYYY ( e.g. both the formats are present in the log shared by you i.e. 19-04-2013 and 07-Jan-2003)
You should Just be able to set the Time Pattern with Separators to D-M-Y as that will read 1 digit, 2 digits, or Name for Month.
This way:
12 = December
and
Dec = December
It seems to me as well, that it is a data inconsistency issue. You have 2 different formats for dates.
Can you try to have one date format, then create an appropriate date mapping and then try to load.
