When you load data into a numbered list the primary key needs to be the "code".
You have it marked as the "name"
Remember, a numbered list has a system generated ID assigned to it so Anaplan cannot map your file to a "name" since name can be duplicated. However, the code is always unique.
See if that helps.
As far as the role mapping goes, that looks right to me.
