Earlier in the day we created a revision tag, and migrated to production with no issues. We are trying to add another revision tag and that action is grayed out. Any idea why this is grayed out?

Has any new development occurred since the last revision tag? If not, it would be the same as the previous and therefore grayed out. To change this, simply add something new to make the revisions different.
